Private Practice Policies To Be Aware Of

  • Telehealth visits require that you upload a form of ID such a driver’s license and you must provide a credit card to be kept on file (not billed until time of service)
  • You must agree to and opt in to receive email/text messages. This is a requirement due to the nature of telemedicine. If you do not consent and agree then please seek in person care.
  • Appointment times are *approximate*. Because pre-registration takes time you may not be seen exactly at your appointment time but we make every effort to stay on schedule.
  • If you are not present in the virtual waiting room at the time of your appointment then a stand-by walk-in patient may be seen in your time slot. You would then be bumped to stand-by status yourself and you can choose to wait or choose to reschedule.
  • No medication refills without a visit.Generous refills are provided where medically appropriate but we still need to document progress and safety in the medical record. I have 4 different worksites and refill requests from pharmacies are hard to trace to the correct practice and will routinely be denied. Please plan to follow up with me or your primary care provider BEFORE you run out of medicines.
  • Help me help you -- PLEASE update your pharmacy before your visit. Wrong pharmacy information is the most common cause of treatment delay. You will have to have another visit to change your pharmacy so please ensure your correct pharmacy is in your profile and make sure the pharmacy is open that day (especially on holidays!).
  • You must agree to electronic communications for this practice as all communication will be electronic. Pre-visit triage questions can be anonymous through the public SelfPayMD.com website—DO NOT post any personally identifiable information on SelfPayMD.com as everything posted there is public.  Medical visits will be though HIPAA compliant synchronous audio/video conferencing using SimplePractice.com. Generally speaking if you have questions after your visit you must schedule another follow up visit for further discussion.  In any event, this practice is structured as urgent care and there is no phone coverage or after hours coverage.
  • No refunds. A telemedicine visit is not a guarantee of a desired prescription. You must still pay for the consultation even if you don’t receive a requested prescription or if you are referred to in person care due to the nature of your condition. Please ask first in the forums whether your case is appropriate to telemedicine, this can be done anonymously.
  • By proceeding you agree and understand that no doctor patient relationship is formed until you are seen in the HIPAA compliant patient portal. Anonymous triage inquiries on SelfPayMD.com do not constitute a doctor patient relationship.
